File a Claim

There are two ways for filing an insurance claim - phone or online.

Submit Claim by Phone

Submit Claims / Forms Via Phone: Flowers and CMS Independent Distributors/Contractors can report claims to our office via phone by calling: 1-866-621-1770.

Submit Claim Online

Flowers and CMS Independent Distributors/Contractors can report claims or submit forms online by using our online form submission. You must be a registered user and logged in to submit these forms.

If you have not registered, please register now. You will be required to have a valid email address in order to complete the registration process.

The After Hours telephone system will direct you on how to file a claim directly to the insurance company. Any accident involving an injury should be reported as soon after the accident as possible to either our office during business hours or directly to the insurance company outside business hours. We monitor the status of the claims submitted by our insured clients.
